With game strength, South will bid 'up the line' by first showing her diamonds. When partner rebids hearts, there is still a small chance that she has a spade suit as well. It is precisely for this situation that 1♠ is an exception to the normal Fourth Suit Forcing convention: it is natural. North might choose to bid 2♠ rather than 3♠, but game in spades should be reached easily enough.
