A strong hand with four spades and six hearts, begins with a transfer to hearts and then forces to game by rebidding 3♠.
Opener assumes you have five cards in the suit you transferred to, and four cards in the second suit. (She doesn't know yet that you have six hearts.) She proceeds as follows:
4+ spades => 4♠
3+ hearts => 4♥
3 spades and 2 hearts => 3NT
If opener bids 3NT, you convert to 4♥.
